This form will help you keep a record of your CE. The Alabama State Board of Veterinary Medical Examiners says you must keep a record of your credit hours and proof of your attendance. CE audits for practicing licensed veterinarians will be conducted at the time of premises evaluation. Each year, your will be required to certify the number of credit hours on a form provided by the Board. The Board form will be mailed to each veterinarian at the time of annual renewal. You may then transfer information from the downloadable form on this page to your Board-provided form.
